Unusual suspect in a common diagnosis: think beyond ‘minimal’

Nephrotic syndrome is a glomerular disease commonly seen in paediatric population. The characterized by hypercoagulable state and risk of thromboembolic complications. Cerebral venous thrombosis (CVT) rare complication children associated with nephrotic an incidence 0.67/100000 very few case reports described. 12-year-old girl steroid dependent syndrome- minimal change disease, background recent COVID-19 infection, presented vomiting, headache, altered sensorium episode generalized tonic clonic seizure. Diagnosis established magnetic resonance imaging (MRI) brain MR venogram which showed superior sagittal sinus thrombosis. Child was started on anticoagulant therapy, gradually recovered no neurological sequelae. Thrombosis should be suspected the added infection. Cortical vein thrombosis, even though rare, considered any patient who presents signs or symptoms, especially this era pandemic. This highlights importance suspecting recognizing post COVID hypercoagulability as triggering factor for complications same. Thromboembolic events are patients but high index suspicion maintained children, particularly those comorbidities.
